How to talk to a hospital social worker

Quick checklist

  • Ask what discharge options are realistic (home health, rehab/SNF, equipment).
  • Ask what insurance will cover and what paperwork is required.
  • Ask for printed instructions and who to call after discharge.
  • Write down names, dates, and callback numbers.

Questions to ask (copy/paste)

  • What are the discharge options and the expected timeline?
  • What services/equipment can be ordered (home PT/OT, walker, hospital bed)?
  • What will insurance cover, and what needs prior authorization?
  • Who coordinates follow-up appointments and transportation needs?
  • If we can’t safely do this at home, what are the alternatives?

Bring this info with you

  • Medication list, allergies, baseline abilities, and who lives in the home.
  • Insurance cards and primary care provider information.
