Quick checklist
- Ask what discharge options are realistic (home health, rehab/SNF, equipment).
- Ask what insurance will cover and what paperwork is required.
- Ask for printed instructions and who to call after discharge.
- Write down names, dates, and callback numbers.
Questions to ask (copy/paste)
- What are the discharge options and the expected timeline?
- What services/equipment can be ordered (home PT/OT, walker, hospital bed)?
- What will insurance cover, and what needs prior authorization?
- Who coordinates follow-up appointments and transportation needs?
- If we can’t safely do this at home, what are the alternatives?
Bring this info with you
- Medication list, allergies, baseline abilities, and who lives in the home.
- Insurance cards and primary care provider information.
